Case Manager

Department: At Promise - Zone 3
Location: Atlanta, GA

Position Summary

This position consists of rehabilitative, environmental support and resources coordination considered essential to assist a consumer/family in gaining access to necessary services and in creating environments that promote resiliency and support the emotional and functional growth and development of the consumer/family

Role and Responsibilities

  • Assist with youth referred from the Atlanta Public Schools, Department of Juvenile Justice, Probation Office, Atlanta Police Departments, Fulton County Courts, and youth appointed for service by Therapists.
  • Displaying positive role-modeling to members
    1. Promote self-esteem
    2. Promote healthy boundaries
    3. Promote good hygiene and appropriate personal appearance among young adults
    4. Reinforce socially acceptable behaviors (ex: manners, eating habits, personal safety)
    5. Teach and reinforce thoughtful decision making
    6. Teach and reinforce professionalism in work tasks and employment
  • Assessing members' history of trauma, strengths and needs.
    1. Administer Ansell Casey Life Skills Assessment (ACLSA)
    2. Administer CANS (Child & Adolescent Needs & Strengths)
    3. Aware of ACE score and implications on service planning
    4. Develop & monitor Individualized Service Plans
  • Coordinating Services and Collaboration
    1. Transport member to and from facility and/or community sites
    2. Meet with assigned member at minimum monthly
    3. Monitor, schedule, facilitate and/or participate in Team Meetings for members.
    4. Collaborate and communicate with other programs in the CHRIS 180 continuum in the provision of services to members
    5. Collaborate and consult with other providers and entities in the provision of best care services to members    
    6. Monitor the provision of services provided
    7. Provide transportation of youth as needed or required.
  • Management of Caseload
    1. Make necessary partnerships to meet member's health, well-being, education, mental health, and employment and future planning needs (i.e., Department of Public Health, Department of Family & Children Services, Georgia Department of Labor, employment staffing agencies, local colleges, and tech schools, etc.)
    2. Research and/or attend trainings to educate self on best practices to provide adequate services to members    
    3. Employ tools to maintain compliance with governing agencies or entities (i.e., DBHDD)
    4. Manage budget and ensure appropriate spending
    5. Oversee member’s progress and program participation
  • Facilitating Skill Building
    1. Research and develop skill building activities to facilitate members’ learning related to life skills, mental health resiliency & substance use recovery
    2. Facilitate skill building discussion individually and/or in groups (ex. Seven Challenges, ATRIUM model)
    3. Facilitate Diversion Groups
  • Documentation Requirements
    1. Provide member updates in Case Notes
    2. Complete Incident Reports as needed prior to shift end
    3. Monitor completion of deadlines (i.e., Monthly Summary Reports)
    4. Provide reports to governing/referral agencies regarding participation of member
  • Performance and Conduct
    1. Maintain confidentiality of member information
    2. Maintain appropriate professional boundaries
    3. Follow the CHRIS 180 Code of Ethics
    4. Follow mandated reporting requirements of GA Code
    5. Support and maintain an environment of emotional and physical safety for members, staff, and other stakeholders always
  • Demonstrate customer service.
    1. Promote positive image of CHRIS 180 programs at all times
    2. Work as a contributing and collaborative team member
    3. Respond to consumer and peer needs in a timely fashion
    4. Answer telephone calls and emails accordingly
    5. Respond to requests for information from stakeholders in a timely fashion
  • Maintain Personal Information and Training.
    1. Attend and participate in staff meetings as scheduled
    2. Attend and participate in required trainings as scheduled
    3. Maintain required certifications
    4. Review, understand and responsible to program policies
    5. Provide documents in a timely fashion to Human Resources
    6. Report changes in status to HR as required by policy

Qualifications/Competencies

  • An understanding of the concepts of childcare,
  • Experience working with youth and transition age young adults with mental health and/or substance abuse diagnosis (co-occurring)
  • Bachelor’s Degree* in the field of social services (Ex. Psychology, Social Work, Sociology)
  • Skilled in assisting and maintaining consumers in their family homes and/or community, 
  • Ability to effectively relate to consumers, parents/custodians of all social, economic, ethnic, and educational backgrounds, with firmness, fairness, and concern,
  • Solid decision-making ability as related to the safety and well-being of consumers, families, community, and staff,
  • Be guided by precedent and established policy and procedure in decision-making. This person will be required to analyze and react to difficult situations, often while under considerable stress and pressure,
  • Dependability, emotional stability and physically capable. This position requires some physical functions that are essential to satisfactory performance of the outlined job responsibilities, i.e., sitting, standing, walking, driving, speaking, hearing and the ability to physically restrain (when necessary),
  • Ability to recognize when assistance and consultation is needed.
  • Ability and/or experience in working with severely emotionally disturbed children, adolescents and their families Must be free to travel as the position demands,
  • A valid driver’s license in the State of Georgia
  • Must be at least twenty-one (21) years of age

  • Technical expertise that allows well informed decisions, usage of available tools and resources and training to execute job responsibilities

  • Ability to collaborate with other staff to ensure best practices and sound decisions
  • Ability to communicate using appropriate tone when corresponding verbally or written (as well as proper usage of grammar and spelling)
  • Ability to adapt and adjust to course and be open to new ideas
  • Ability to manage workload by having a clear understanding of task/project and set realistic timelines and goals

Schedule

This is a full-time position and typically works Monday – Friday 11am to 8pm.
