Resource Navigator

Department: Community Initiative
Location: Atlanta, GA

Position Description

The primary duties include but are not limited to the following:

  • Aid clients in connecting the client with resources to meet needs identified on needs assessment.
  • Diligently track all referrals, forms, and outcomes as it relates to services/resources provided.
  • Plan in a proactive manner to assist the client and/or family in connecting them to local services.
  • Work collaboratively with representatives from partner organizations to ensure communication and service delivery is effective.
  • Provide service and resource coordination to assist the client and/or family in gaining access to necessary rehabilitative, medical, social, and other services and supports (E.g., Medicaid, SSI, consents, etc.)
  • Assist clients with making appointments for services that meet their needs.
  • Provide follow-up contact with contact following their scheduled appointments. Solicit feedback regarding their appointment. Track all feedback and surveys.
  • Assure clients are actively contacted and encouraged to participate in services.
  • Collaborate 100% with other agency staff to accomplish work related activities.
  • Document services and contacts with each in an excel spreadsheet or in EHR.
  • Maintain a positive attitude toward all clients and their families 100% of the time.
  • Attend and participate in scheduled staff meetings, administrative meetings, and in-service trainings 100% of the time.
  • Abide by all rules of confidentiality and the Code of Ethics prescribed by CHRIS 180 100% of the time.
  • Positive attitude, attention to detail and phone skills needed.
  • Will work closely with the other clinical staff at CHRIS 180 including Therapists, Community Behavior Specialists, and others as indicated.
  • Practice Requirements
    • Adhere to the organization policies and procedures.
    • Attend individual supervision weekly.
    • Attend treatment team meetings.
    • Participate in clinical consultation.
    • Review case studies.
    • Provide feedback for hard to manage cases.
  • Performance and Conduct
    • Maintain confidentiality of consumer information
    • Maintain appropriate professional boundaries.
    • Follow the CHRIS 180 Code of Ethics
    • Follow mandated reporting requirements of GA Code
    • Support and maintain an environment of emotional and physical safety for consumers, staff, and other stakeholders always.
  • Demonstrate customer service
    • Always promote a positive image of CHRIS 180 programs.
    • Work as a contributing and collaborative team member
    • Respond to consumer, referral, and peer needs in a timely fashion.
    • Answer telephone calls and emails accordingly
    • Respond to requests for information from stakeholders in a timely fashion.
  • Maintain Personal Information and Training
    • Attend and participate in staff meetings as scheduled.
    • Attend and participate in required training as scheduled.
    • Maintain required certifications.
    • Review, understand and be responsible for program policies.
    • Provide documents in a timely fashion to Human Resources
    • Report changes in status to HR as required by policy

Qualifications

  • Bachelor’s degree in social work or related field; certification in social services and/or social services experience preferred.
  • Flexible Hours, occasional evening, or Saturday community events.
  • Excellent time management skills.
  • Comfortable doing community outreach.
  • Current CPR and First Aid certification

  • Ability to manage workload by having a clear understanding of task/project and set realistic timelines and goals.

Schedule: This position typically works Monday through Friday with occasional evenings and Saturdays to attend community events. This position requires flexibility. This position will primarily be based out of a Dekalb County School and will require some light travel.
